acknowledged that parents have read the list of
Augusta West Dance Studio’s Rules and Policies
provided with registration material and agree to
honor them.
• Registration fee is non-refundable.
• Augusta West Dance Studio, Inc. may cancel
classes for any reason, in which event tuition
and other fees paid shall be refunded pro-rata.
• Dancing is an enjoyable form of exercise,
which among many advantages promotes good
posture, flexibility, strength, and
gracefulness. However, like any physical
activity, there naturally exists the risk of
injury. Upon signing this registration form, it
is understood by all parties involved that
neither Augusta West
Dance Studio, nor Dianne Clements or
Cindy Folger or any other instructor will be held
responsible in the event of an injury. Please
keep your child’s instructor informed of any
physical or developmental needs which may
require special attention.

Augusta West Dance Studio Rules and Policies:
• Please observe
proper dress/hair code.
• Please bring
children to class on time.
• Please pick up
children promptly after class and advise teacher if
someone else will be
picking up your child.
• Due to the limited
capacity of our lobby, we ask people to please not
come more than 15
minutes early and to
not linger around after classes are over.
• Studio is not
responsible for items left in
dressing area or any
other area of the studio. Do not leave valuables
unattended.
• Please leave
strollers folded at the end of the upstairs hallway.
• Children must be
accompanied by an adult and wait quietly until
called into class by teacher.
• Please keep voices
low in the waiting area and be courteous to other
people’s ears if you need
to use your cell
phone.
• Students should
please use bathroom before class.
• Teachers will admit
and dismiss students from the studio.
• Please do not open
studio doors while classes are in session.
• Parents may observe
class once a semester
• No street shoes in
the studio.
• No food or drinks in
the studio.
• Please do not leave
diapers or smelly food in our garbage cans.
• Make-up classes are
limited. Make-up classes must be scheduled with the
instructor and done
within the semester of your child’s absence.
• Make-up classes may
be done in the same level class on a different day.
• Classes will not be
prorated.
• Dance classes missed
due to late registration will be not be pro-rated.
• AWDS does not issue
drop-out tuition refunds after the second week of
each school-year
semester.
• Augusta West Dance
Studio reserves the right to cancel classes due to
low
registration. In the
unlikely event that a class is cancelled, every
effort will be made to switch
the student to an
appropriate alternative class. If this is not
possible tuition will be refunded.
• In the event of a
teacher’s absence, AWDS will do everything possible
to find another faculty
member or qualified
outside substitute to fill in. AWDS is not
responsible for notifying
parents of
substitutes.
Recital costume orders
are placed at the beginning of each semester.
Costumes are therefore
ordered a bit larger
to leave room for growth. Parents may need to make
minor alterations to
recital costumes
